Terms of Services
Green Medical and Consulting, LLC
Effective Date: June 2022
Last Updated: October 27, 2026
1. Acceptance of Terms
These Terms of Service govern your use of the website, appointment scheduling services, telehealth services, and related healthcare services provided by Green Medical and Consulting, LLC (“Clinic,” “we,” “our,” or “us”).
By using our website, requesting an appointment, or receiving services, you acknowledge that you have had an opportunity to review these terms.
Additional patient consent forms, financial policies, telehealth consent forms, and clinical agreements may apply. Where required, those documents must be completed separately.
2. Scope of Services
The Clinic provides medical evaluations related to eligibility for medical cannabis under applicable state law. Services may include:
-
Review of medical history and available records.
-
Clinical evaluation of qualifying conditions.
-
Discussion of potential benefits, risks, contraindications, and alternative treatment options.
-
Medical cannabis recommendations, certifications, or other permitted documentation when clinically appropriate and legally authorized.
-
Follow-up evaluations and education concerning safe use.
-
Telehealth consultations, where permitted.
The Clinic does not guarantee approval for a medical cannabis program, issuance of a state registry card, acceptance of a certification by a regulatory agency, or any particular clinical outcome.
The final decision to provide a recommendation or certification rests with the authorized clinician following an appropriate evaluation.
3. Eligibility and Patient Responsibilities
You agree to:
-
Provide accurate and complete demographic, medical, medication, and relevant treatment information.
-
Disclose allergies, significant health conditions, and other information necessary for safe clinical decision-making.
-
Provide requested medical records and documentation.
-
Notify the Clinic of material changes in your health status.
-
Comply with applicable state medical cannabis program requirements.
-
Treat Clinic staff and healthcare professionals respectfully.
-
Provide accurate information about your physical location during telehealth appointments.
You must meet applicable age, identity verification, residency, consent, and other legal eligibility requirements.
The Clinic may postpone or decline an evaluation when required information is missing, eligibility cannot be established, clinical assessment is inadequate, or services cannot legally or safely be provided.
4. No Guarantee of Certification
Payment for an appointment covers the services identified at booking and does not purchase a guaranteed medical cannabis recommendation, certification, or state approval.
A clinician may determine that a patient does not meet the applicable eligibility requirements or that medical cannabis is not clinically appropriate.
Any state application, registry fee, card fee, or other third-party charge is separate unless expressly stated otherwise.
5. Telehealth Services
Telehealth services are provided only when permitted by applicable law and when clinically appropriate.
Before receiving telehealth services, you may be required to provide informed consent and confirm your identity and current physical location.
You understand that telehealth may involve limitations, including the inability to perform certain physical examinations or obtain immediate diagnostic testing. A clinician may recommend an in-person evaluation or referral when needed.
You are responsible for using a suitable device, maintaining a reliable internet connection, and participating from a reasonably private location.
The Clinic may discontinue or reschedule a telehealth appointment if technical difficulties, privacy concerns, patient safety issues, or legal restrictions prevent an adequate evaluation.
Telehealth availability does not mean the Clinic is authorized to treat patients located in every state. Services are limited to jurisdictions where the clinician and Clinic are legally permitted to provide them.
6. Medical Cannabis Risks and Patient Education
Medical cannabis may cause adverse effects, including dizziness, impaired coordination, sedation, anxiety, changes in cognition, and other effects that vary by individual and product.
Potential risks may be greater for certain patients, including those with specific cardiovascular, psychiatric, respiratory, or other medical conditions, or those taking medications that may interact with cannabis.
Patients should discuss their complete medical history and medication list with their healthcare professionals.
A medical cannabis recommendation or certification does not guarantee safety, effectiveness, or freedom from adverse effects. Patients should follow applicable program rules, product labeling, and individualized clinical guidance.
Patients must not drive, operate machinery, or engage in activities requiring unimpaired judgment while impaired by cannabis. Cannabis use during pregnancy or breastfeeding may pose health risks and should be discussed with a qualified healthcare professional.
7. No Emergency Services
The Clinic is not an emergency medical service and does not provide continuous monitoring or guaranteed immediate responses to messages.
For a medical emergency, call 911 or go to the nearest emergency department. Do not rely on appointment requests, email, text messages, or the patient portal for urgent assistance.
8. Appointments, Cancellations, and No-Shows
Appointments must be scheduled through the Clinic's designated scheduling system or authorized staff.
Patients should arrive on time for in-person appointments and be ready to connect at the scheduled time for telehealth visits.
Cancellation, rescheduling, late-arrival, and missed-appointment policies will be communicated before booking.
Cancellation policy:
Appointments cancelled or rescheduled with less than 24 hours’ notice, as well as missed appointments without prior notification, will incur a $25 cancellation fee. This fee is intended to compensate for reserved appointment time and is separate from any charges for services already provided.
To avoid this fee, please contact our office at 662-912-6399 or kpeggins@gmedicalclinic.com at least 24 hours before your scheduled appointment.
No-show policy:
Patients who fail to attend a scheduled appointment without prior notice will be considered a no-show and charged a $25 fee. This fee is separate from any charges for medical services already provided.
Please contact our office at 662-912-6399 or kpeggins@gmedicalclinic.com if you need to cancel or reschedule your appointment.
We understand that emergencies and unexpected circumstances can occur. Exceptions to this policy may be considered on a case-by-case basis.
The Clinic may cancel or reschedule appointments due to clinician availability, technical difficulties, emergencies, or other circumstances. When appropriate, patients will be notified and offered available alternatives.
9. Fees and Payments
Fees for evaluations, follow-up appointments, documentation, and other services will be disclosed before the applicable service is purchased.
Payment is due according to the Clinic's stated payment policy. The Clinic may require payment before an appointment or at the time services are rendered.
Unless expressly stated otherwise, third-party charges, including state registry fees, are not included in Clinic fees.
Refund policy:
At Green Medical, appointment fees are subject to our cancellation and refund policy. Patients who cancel or reschedule their appointments at least 24 hours before the scheduled appointment time may be eligible for a full refund of any prepaid appointment fee.
Appointments cancelled with less than 24 hours’ notice are nonrefundable, and a $25 cancellation fee may apply. Missed appointments are also subject to the Clinic’s no-show policy.
Refunds, when approved, will be returned to the original method of payment. Processing times may vary depending on the payment provider.
Exceptions may be considered on a case-by-case basis for emergencies or other extenuating circumstances. This policy does not apply to fees that are refundable under applicable law.
No fee will be described or treated as payment for a guaranteed clinical decision or state approval.
10. Medical Records and Privacy
The Clinic maintains patient records in accordance with applicable privacy, security, and medical record retention laws.
Your health information may be used or disclosed for treatment, payment, healthcare operations, or other purposes permitted by law.
Please review the Clinic's Privacy Policy and, where applicable, its separate Notice of Privacy Practices for additional information.
Nothing in these Terms of Service waives your rights under applicable privacy or healthcare laws.
11. Website Use and Intellectual Property
The content on our website is provided for general informational and administrative purposes. Unless expressly stated otherwise, the Clinic owns or has permission to use its website text, branding, graphics, and other materials.
You may not reproduce, distribute, modify, or commercially exploit Clinic materials without prior written authorization, except as permitted by law.
You agree not to use our website to submit fraudulent information, interfere with website security, impersonate another person, or engage in unlawful activity.
12. Third-Party Services
The Clinic may use third-party services for scheduling, telehealth, payment processing, electronic records, and state program applications.
Your use of those services may also be governed by the third party's terms and privacy practices. The Clinic does not control independent third-party systems and cannot guarantee their uninterrupted availability.
13. Limitation of Website Information
We make reasonable efforts to maintain accurate website information. However, website content may not reflect the latest medical research, state program requirements, or changes in law.
Website content is not a substitute for individualized medical advice, diagnosis, or treatment.
Nothing in these terms excludes or limits liability where such exclusion or limitation is prohibited by law, nor does it waive applicable patient rights.
14. Suspension or Termination of Services
The Clinic may decline, suspend, or discontinue services when permitted by law and consistent with professional and ethical obligations.
Where applicable, the Clinic will provide appropriate notice, referrals, continuity-of-care arrangements, and access to records as required by law.
Termination of services does not eliminate outstanding payment obligations for services already provided.
15. Governing Law
These Terms of Service will be governed by the laws of [State], to the extent permitted by applicable law.
Any dispute will be handled in a manner consistent with applicable state and federal law. Nothing in these terms limits a patient's right to file a regulatory or professional complaint.
16. Changes to These Terms
We may update these Terms of Service periodically. Revised terms will be posted on our website with an updated effective date.
Changes will apply prospectively unless otherwise permitted by law. Where legally required, we will obtain additional consent or provide notice before implementing material changes.
