***PLEASE READ PRIOR TO BOOKING*** MUST BE 18 YEARS OF AGE TO BOOK ***A cancellation fee of $50 will be charged with less than 24 hour notice or a no show*** You much have one of the 21 qualifying medical conditions listed on the MS State Department of Health website in order to qualify. ***You will need medical records to support the qualifying medical condition. This is an in-person evaluation at the clinic for a medical cannabis card certification. You will receive information on how cannabis may help with your qualifying condition, what types of cannabis may be best suited to your condition, and what are the most effective ways of using medical cannabis to assist in management of your symptoms. Your Green Medical patient forms must be completed 24 hours prior to your appointment. ****Green Medical Forms will be sent via email and text for completion****

Cancellation Policy

A fee of $50 will be charged if cancellation not made within 24 hours or more
